The 49ers have finally announced that quarterback Alex Smith will undergo season-ending surgery to correct his separated right shoulder. The Sacramento Bee reports it is not yet known when the surgery will be performed. Smith suffered the injury on September 30 and returned after two weeks off, but then began to suffer spasms in the forearm. The handling of this injury has been a point of contention between Smith and the team, and it may be the beginning of the end for his tenure in San Francisco.

49ers quarterback Trent Dilfer has already been downgraded to doubtful for Sunday’s game against the Cincinnati Bengals. Dilfer suffered a concussion yesterday against the Vikings when he tried to dive headfirst for a first down, but ran into the shoulder of Charles Gordon. Shaun Hill will be the likely starter, but coach Mike Nolan said that the team would be looking for a veteran quarterback this week. The team has also not officially ended the season of Alex Smith , who needs surgery to repair his non-throwing left shoulder.

The 49ers were no closer Friday to setting a timetable or target date for the return of Alex Smith, who is nursing a shoulder injury. "I'm relying on the medical staff," coach Mike Nolan said in the Oakland Tribune. "They said to give him light throwing. He did light throwing. Our planning as coaching staff is based on the medical staff, and we can't go any further than that." Trent Dilfer will start again for the Niners Sunday.

49ers quarterback Alex Smith will spend Tuesday meeting with the esteemed Dr. James Andrews in Alabama. Although the good doctor has been used as a consultant since Smith hurt his shoulder in late September, the San Francisco Chronicle reports this will be the first chance Andrews has had to examine Smith’s injured shoulder in person. Smith still can’t grip the football like he did before the injury and that is impacting his play. The team has claimed he suffered a sprained shoulder.
November 17, 2007
Niners coach Mike Nolan announced earlier this week that Trent Dilfer will start over Alex Smith on Sunday. Smith was going to be the third quarterback, but he'll now be the No. 2, since presumptive backup Shaun Hill dislocated a finger, according to the Contra Costa Times. Smith is dealing with a separated right shoulder and strained forearm.
November 14, 2007
49ers quarterback Alex Smith admitted to media members that his recently separated right shoulder and strained forearm have had a great deal to do with his poor play in the last three games. He even went so far to wonder aloud to the same media members if he should take a prolonged break and prepare his body for 2008.
November 2, 2007
Alex Smith has been practicing this week and should have no problems with his shoulder against Atlanta.
October 26, 2007
Alex Smith has officially been named the starter for Sunday's matchup against the Saints. Smith has been out since Week 4 with an injured right shoulder. "It's not so much my confidence in him, it's his confidence in taking reps (in practice)," coach Mike Nolan told the AP Thursday. "I expect him to play well, and he looked good. He looked healthy today."
October 25, 2007
49ers quarterback Alex Smith, who has been out since Week 4 with an injured right shoulder, was able to practice fully on Wednesday. Unless he suffers a setback during the next two days, Smith will be the team’s starter Sunday against the Saints. However, don’t do anything rash like insert him into your starting lineup.
October 24, 2007
Quarterback Alex Smith is lobbying head coach Mike Nolan for his old job. Smith has made all the throws in practice this week, and says that his injured right shoulder is ready to take hits. Smith took every snap with the first team offense on Wednesday, but Nolan will not name his starter until later in the week.
October 20, 2007
Not only will Alex Smith not start Sunday's game, but he will be third on the depth chart. "It's a matter of reps more than anything," coach Mike Nolan said of the decision to make Smith No. 3, according to the Contra Costa Times. Smith is recovering from a shoulder injury and might be ready to start next week. For now, though, Trent Dilfer will start and be backed up by Shaun Hill.
October 19, 2007
Alex Smith (shoulder) will dress Sunday against the Giants but not start, the AP reports. Smith participated in drills Thursday, in pads, so there's a good chance we'll see him in Week 8, if not in a relief role Sunday.
October 17, 2007
Quarterback Alex Smith participated in individual drills on Wednesday, but acknowledged that he would need extensive practice time on Thursday and Friday in order to be ready to start on Sunday against the Giants. Trent Dilfer has continued to take the first team snaps at this point.
October 16, 2007
49ers quarterback Alex Smith, who hasn’t played since suffering a shoulder injury in Week 4, threw extensively on Saturday and did more tossing Monday. He told the Sacramento Bee that his velocity is close to normal and that his longest throws traveled about 55 yards. Head coach Mike Nolan has called Smith “questionable” for this week. Fantasy owners should be avoiding him since he was not being productive even before the injury.
October 11, 2007
49ers quarterback Alex Smith threw the ball around during Wednesday's practice and head coach Mike Nolan continues to be cautiously optimistic that he’ll be able to return after the team’s bye this week. "I would say he's questionable," Nolan told the Sacramento Bee. "Because 'hopeful' is not one of the choices." Smith isn’t a fantasy option unless you’re in a ridiculously large league, but his return may help open things up a bit for Frank Gore and allow Darrell Jackson to remember how a football feels.
October 9, 2007
49ers quarterback Alex Smith was able to throw from 25 yards on Monday and hopes to be under center after the team’s Week 6 bye. "I pushed it back a little bit," he told the San Francisco Chronicle. "I was throwing about 25 yards. I wasn't gunning them in there. It definitely felt better than yesterday."
October 8, 2007
Alex Smith was able to toss a ball lightly on the sideline Sunday from about 8-12 yards. Although that's a good sign, coach Mike Nolan said that Smith "from a medical standpoint" is questionable for their next game, according to the Contra Costa Times. The Niners have a bye this week, so Smith will have extra time to recover before they face the Giants in Week 7. If Smith isn't ready by then, Trent Dilfer will be under center again.
October 3, 2007
No news is good news for quarterback Alex Smith and his right shoulder. The Niners sent the MRI results to noted orthopedic surgeon, Dr. James Andrews, and his diagnosis was the same as before, a moderate shoulder separation that will not require surgery. Smith is listed as out for this week’s game, and he will have to wait for the swelling to subside in his shoulder before further evaluation or treatment.
October 3, 2007
It turns out the 49ers are not seeking a second opinion on Alex Smith’s injured shoulder. The Sacramento Bee is reporting that Smith will not head to Alabama to see Dr. James Andrews, but instead a copy of Smith's MRI results will be sent to the good doctor. The paper calls this a standard procedure. While this means Smith’s season is probably not over, it will be several weeks before he can return to the field. That will give Trent Dilfer plenty of time to remind us that he’s terrible and over the hill.
October 3, 2007
Alex Smith is seeking a second opinion from Dr. James Andrews after preliminary MRI results indicated he appears to have suffered a serious injury to tendons in his right shoulder in Sunday's game, a team source told ESPN.com. If the diagnosis performed at Stanford is confirmed by Andrews, the source said that it is expected Smith would opt to have corrective surgery immediately even though he would likely miss the rest of the season.
October 2, 2007
The 49ers won’t have Alex Smith under center for a long time. The quarterback told the San Francisco Chronicle that he expects to miss several weeks with the Grade 3 separation of his throwing shoulder he suffered on Sunday. Trent Dilfer will start in Smith’s place, and that is bad news for the passing game.
October 1, 2007
The 49ers will be without their starting quarterback for at least one week. The Associated Press is reporting that Alex Smith will not play against the Ravens because of the separated shoulder he suffered in the first quarter of Sunday's loss to the Seahawks. No timetable has been set for his return, but he will not need surgery. Smith's injury will probably sideline him for at least one month, and possibly much longer. Of course, he was playing so badly that he was of no use fantasy owners even before he got. Losing Smith is a blow to the entire 49ers offense, especially Darrell Jackson, who had his best day as a Niner in the last full game Smith played. Trent Dilfer will start in Smith's place.
October 1, 2007
Alex Smith suffered a separated right shoulder during Sunday's game and did not return. Coach Mike Nolan said he considered Smith to be doubtful for Week 5 against the Ravens, according to the Contra Costa Times. Trent Dilfer replaced Smith and was pretty awful (12-of-33 for 128 yards and two interceptions), so he wouldn't be an appealing option against the Ravens.
September 30, 2007
Alex Smith left Sunday's game in the first quarter with an injured right shoulder. His return is questionable, according to the AP. Veteran Trent Dilfer replaced Smith.
